Our Registered Nurses, Physical Therapists, Occupational Therapists, Speech Therapists and Social Workers coordinate care within the entire health care delivery system, working with institutional providers and primary physicians to continuously assess medical conditions; ensure medication compliance by educating patients and caregivers on the purpose, frequency, proper administration, and possible side effects of medications; while maintaining a level of care, compassion and consistency that dramatically improves the quality of life of our patients.

Based on your physician’s orders and our evaluation, our licensed personnel can provide you with the following services at no cost to you:

Skilled Nurse

Will monitor your overall status and report to your physician. Additionally, your nurse will monitor medications, do lab work, perform wound care, maintain specialized needs and provide instruction on diet and safety measures.

Home Health Aide

Will provide personal care and assistance with your daily living activities.

Occupational Therapist

Will use specialized techniques and equipment to restore your ability to function optimally.

Physical Therapist

Will provide physical and functional rehabilitation in ambulation, safe transfers, prosthesis training and cardiac/respiratory rehabilitation.

Speech Therapist

Will assist in restoring speech and swallowing ability.

Social Worker

Will provide information on state funded and private agency programs that offer assistance in finances, housing, medications, meals, as well as homemaker and companionship programs.
